So I've been trying so hard to have a flat tummy (it might be hard to tell from some of my pics but that pooch of fat is deff there). I was on a workout plan of Insanity 3x a week for cardio and lifting legs/abs, chest/tri, bi/back/shoulders 3x a week. I would eat egg whites or plain oatmeal for breakfast, have a lean 1 smoothie for lunch, and something like a tiny piece of chicken and vegetables for dinner. Every now and then i'd have a snack of a protein bar or fruit.

But that belly fat still persists. HELP! What am I doing wrong? What could I do differently? It's been months and I've gained muscle weight but that's it.

Cliffs:
I diet.
I exercise.

Firstly, if you are doing Insanity, you have to make sure your body has enough fuel to burn, as it is HIGHLY intensive!
You may be eating to little calories to be honest. I know it sounds weird, that you may need to eat more to lose weight. In your case though, it may be just that. If you don't fuel your body with a healthy range of fat, protein and carbs, your body will shut down.

Your body is going to make a choice: lose body fat or lose muscle. An inadequately fueled body will select to drop calorie-burning muscle rather than fat. Substantial reduction of lean muscle mass results in weight loss without improvement your body shape or health. This leaves you frustrated as well as ever-battling your weight.

So eat more, so your body can hold on to your muscle, and release your fat.
